HCRS
The Healthworks Assertive Community Treatment (ACT) program provides intensive, integrated services and support to individuals experiencing homelessness or housing insecurity in Windham County. This role focuses on delivering participant-led care through peer support, helping individuals connect with resources, set goals, and advocate for their health and autonomy. The position involves working within a small staff-to-participant ratio to provide 24/7 support and collaborate with community partners.
