Peer Support Specialist

As a Peer Support Specialist on the Assertive Community Treatment (ACT) team, you will provide practical support and advocacy to individuals with serious and persistent mental illness, helping them manage daily challenges and build greater independence. You will work directly with patients in their homes and community settings to strengthen daily living skills, support housing stability, and connect with essential resources. The role involves collaborating with a multidisciplinary team to deliver coordinated, individualized care and crisis support.
