Per Diem Case Manager, Clinical Stabilization Services (CSS)

The Per Diem Case Manager provides care coordination and discharge planning for individuals in residential substance use treatment, working with clinicians, recovery specialists, and community partners to ensure clients access needed services and transition back to the community. The role includes assessments, developing care plans, coordinating referrals, facilitating groups, and conducting post‑discharge follow‑up.
