About this role

Jefferson Health at Home by BAYADA Hospice is seeking a compassionate per‑diem Nurse Practitioner to provide hospice and palliative care to clients in Bucks County, Montgomery County, and Northeast Philadelphia. The NP will conduct home hospice recertification visits, monitor client conditions, execute doctors' orders, and perform assessments as part of a multidisciplinary team.

Responsibilities

Make home face‑to‑face hospice recertification visits
