Permanency Care Mgr

The Permanency Care Manager works with children in care, foster families, and birth families to achieve permanency through coordination of appointments, meetings, and services. The role involves supporting families, facilitating provider meetings, and participating in treatment and discharge planning. The position requires on-site work with some travel and availability for night, weekend, and on-call hours.
