Pharmacist Clinical Coordinator

The Pharmacist Clinical Coordinator works collaboratively with the patient care team to promote the safe and effective use of medications through telephone, outpatient, and home visits. This role involves coordinating medication management for patients enrolled in value-based clinical programs and optimizing medication therapies through clinical assessment. The position also includes a leading role in precepting pharmacy residents, students, and interns.
