Pharmacist PT

The Pharmacist PT works within the health system to accurately fill prescriptions, verify dosages, and check for drug interactions on outpatient orders. This role involves counseling patients on medication use, side effects, and storage, while communicating with providers to support medication adherence during the discharge process. The position also includes conducting medication therapy management to help manage chronic disease states and coordinating patient comfort through palliative care and hospice medication regimens.
