Pharmacy Care Coordinator

The Pharmacy Care Coordinator serves as a patient-facing representative at partner practice sites, introducing and enrolling patients in a specialized mail-order pharmacy program that provides pre-sorted medication packaging and proactive refill management. This role involves engaging with patients to explain service benefits, obtaining consent, and documenting enrollment decisions in the electronic health record. The coordinator also conducts telephonic outreach to patients who cannot be supported onsite, ensuring they have timely access to medications and pharmacist-led clinical services.
