Physical Therapist / Clinical Coordinator

Rehab Without Walls is seeking a Physical Therapist who will also serve as a Clinical Coordinator, delivering one‑on‑one, community‑based neurorehabilitation to adults with brain injury, stroke, spinal cord injury, and other neurological conditions. The role combines direct therapy services with care coordination responsibilities, including developing treatment plans, leading interdisciplinary case conferences, and managing communication with patients, families, physicians, and payers.
