About this role

Provides evaluation, planning, and administration of physical therapy programs to restore function, prevent disability, and help patients reach their maximum level of independence. Works with patients and families to develop and update individualized plans of care and improves quality of life through prescribed exercise, hands-on care, and education. Position supports Home Care assignments and requires travel to patient locations.
