Physical Therapist (PT) – PACE

The Physical Therapist provides individualized rehabilitation services to older adults with complex medical and functional needs as part of an interdisciplinary team. The role focuses on improving mobility, reducing fall risk, and helping participants maintain independence in their daily lives. This position supports a community-based PACE program with services delivered at the center, participants' homes, or other assigned settings.
