Physical Therapist

The Physical Therapist provides skilled physical therapy services on an intermittent basis to clients in their homes, performing evaluations and treatment plans in accordance with physician orders. This role involves assessing client functioning, setting treatment goals, and coordinating with an interdisciplinary team to ensure comprehensive care.
