Physician

Optimal Care is seeking a Physician to provide advanced primary and specialty care to geriatric and medically complex patients, often in their homes or care settings. The role involves conducting comprehensive medical evaluations, managing acute and chronic conditions, and coordinating care with patients, families, and interdisciplinary teams. Physicians will also provide medical leadership and collaborative oversight for Nurse Practitioners and Physician Assistants to ensure high-quality, coordinated care.
