Population Health Coordinator

The Population Health Coordinator supports the FQHC's mission by helping the clinic meet quality and performance targets tied to value-based contracts and payer incentive programs. This role serves as a key liaison between the health center and payers to coordinate performance tracking, reporting, and program compliance. The ideal candidate understands healthcare quality measures and has a strong ability to communicate across clinical, administrative, and payer audiences to ensure the overall wellbeing of the patient population through collaborative engagement.
