About this role

The Post-Acute Network Performance Specialist serves as a performance liaison to post-acute partners, including Skilled Nursing Facilities and Inpatient Rehabilitation Facilities, to support the execution of the enterprise post-acute strategy. This role focuses on conducting structured performance reviews, identifying operational barriers impacting patient throughput and readmissions, and facilitating resolution pathways with internal and external stakeholders. The position operates at the partner-performance level and does not involve inpatient discharge planning or care management workflows.

Responsibilities

Conduct facility-level performance reviews with post-acute partners to analyze throughput and readmission trends
Identify operational barriers impacting patient transitions and coordinate resolution plans with internal and external stakeholders
Facilitate escalation communication and maintain structured performance documentation to support network expansion and governance
