Practice Coach

The Practice Coach provides practice transformation support to Health Center Organizations to help them establish and reach clinical, operational, and quality improvement goals. This role involves coaching clinical teams, facilitating peer learning, and assisting care teams in using data to drive decision-making and adopt evidence-based care pathways. The position requires strong facilitation skills, experience with change management, and the ability to work remotely while occasionally traveling to support on-site implementations.
