Practice Manager (LD 25 +)

The Practice Manager leads and supports a team of Social Workers and Support Workers within the Learning Disabilities Team 25+, providing professional leadership and ensuring high-quality, person-centred care for adults with learning disabilities. The role involves overseeing assessments, reviews, safeguarding, and case management while collaborating with health partners and other agencies to improve service delivery. This position requires a motivated individual with at least four years of post-qualifying social work experience to drive safe practice and positive resident experiences.
