Practice Transformation Manager

The Practice Transformation Manager serves as the primary point of contact between Vytalize Health and assigned provider practices, leading the on-the-ground implementation of value-based care programs. This role drives practice-level adoption of population health management strategies, clinical workflow improvements, and quality performance initiatives to improve patient outcomes and advance VBC contract performance. The position requires building strong relationships with practice administrators, providers, and staff to drive collaboration and sustainable change.
