About ADAPT Community Network

adaptcommunitynetwork.org

ADAPT Community Network is a not‑for‑profit human services organization that empowers people with disabilities and others facing barriers to independence through innovative, person‑centered programs. They offer a spectrum of supports—including employment services (e.g., Project Connect), community living and housing supports, behavioral health, and family/community inclusion—designed to move people "from obstacles to opportunities" and "from paycheck to purpose." As a pioneer in cutting‑edge service models, ADAPT focuses on individualized planning, coordination of services, and measurable outcomes. Individuals, families, and partner agencies rely on ADAPT for comprehensive supports and help navigating programs and applications.
