Prevention Case Manager

The Prevention Case Manager assists families by implementing individualized service plans and facilitating relationships between families, agency personnel, and other service providers. This full-time role involves providing in-home casework, crisis intervention, and coordination of clinical, financial, and community support services. The position requires managing Article 10 cases and serving as a liaison between families, courts, and other agencies to ensure positive outcomes.
