Primary Care Integrated Behavioral Health Clinician

The Integrated Behavioral Health (IBH) Clinician provides brief behavioral health assessment, triage, and treatment embedded within primary care. The clinician works closely with primary care teams to address mental health and substance use concerns, deliver crisis intervention, and coordinate referrals to appropriate services. Services are provided in person, telephonically, and via telemedicine to support patient stability and navigation of the healthcare system.
