Primary Care Nurse Practitioner

The role is a full‑time hybrid Primary Care Nurse Practitioner position providing outpatient care to pediatric and adult patients across two family medicine practices. The NP will spend two days per week in the Clinton Street clinic and three days per week delivering telehealth services for both Clinton Street and Windsor locations, handling acute and chronic conditions, preventive care, and medication management.
