Primary Care Physician

This role involves providing preventive primary care for adults aged 65 and older, with a focus on Medicare Advantage, chronic disease management, and preventive care. Physicians will see approximately 12–15 patients per day with 20–40 minute visits within an integrated care model. The position offers value-based care training and collaboration with a comprehensive support team including NPs, PAs, and social workers.
