This full-time outpatient position focuses on senior care within a value-based model, serving as a Pod Leader within an interdisciplinary team. The role offers a smaller patient panel to foster deeper, long-term relationships and includes a Monday–Friday schedule with home visits and rotating phone-only call coverage. The position provides growth opportunities into leadership and medical director roles.
