A compassionate and team-oriented Primary Care Physician is sought to join an integrated care network in Pueblo, CO, providing comprehensive, patient-centered medical care to individuals and families across the lifespan. The role focuses on preventive, chronic, and acute care services while supporting the mission to improve access to quality healthcare for underserved populations. The physician collaborates within an integrated care model alongside behavioral health, perinatal, dental, pharmacy, nursing, and care coordination teams to promote whole-person care and improve patient outcomes.
