Primary Care Physician

Conviva Primary Care, a subsidiary of Humana Inc., is seeking a Primary Care Physician to work on-site at their Palm Coast, FL clinic. The role involves providing proactive, preventive care to seniors, including wellness visits, chronic condition management, and screenings, while addressing social, emotional, and behavioral needs. The physician will report to the Associate Medical Director and see an average of 15 patients per day with longer appointment times.
