WelbeHealth is seeking a Primary Care Provider with a psychiatric-mental health focus (NP/PA) to support seniors in the PACE program so they can continue living in their homes and communities. The role partners closely with PACE physicians and an interdisciplinary team to support whole-person behavioral health care across settings, including in-person and telehealth.
