Primary Care Team Nurse (RN/RPN)

Vancouver Coastal Health is seeking a Primary Care Team Nurse (RN/RPN) to join the Complex Care Housing team in Sechelt, BC. The role involves practicing within an integrated, team-based primary care and population health model to coordinate and develop care plans for clients with complex health needs, including chronic disease and mental health challenges.
