Program Manager – Patient Transitions

The Program Manager – Patient Transitions provides operational and people leadership for a 14‑bed unit, recreation therapy services, and a portfolio of patient transition programs across the Alternative Health Facility, Kingston General Hospital, and community settings. Reporting to the Program Operational Director, the role oversees program integration, staff leadership, quality and financial performance, and ensures safe, coordinated, patient‑centred care.
