Program Manager- Waiver Case Management

The Program Manager- Waiver Case Management provides person-centered case management services to individuals with serious mental illness, substance use disorders, and co-occurring medical conditions eligible for CADI or Brain Injury Waiver services. This role involves leading and supervising a team, managing a caseload, and coordinating with healthcare professionals to develop and update client care plans. The position requires ensuring financial authorizations, maintaining accurate records, and providing crisis assessment and intervention to support client safety and well-being.
