Project Manager - Rural Access & Care Coordination

The Project Manager - Rural Access & Care Coordination leads the implementation of the RHT Initiative 6.2 and CCBHC Bridge Funding plan, translating grant scope into operational practice. This role coordinates internal teams, vendors, and community partners to execute rural access, care coordination, and sustainability activities within approved timelines and budgets. The position reports directly to the Executive Vice President of Outpatient Services and ensures alignment with grant deliverables and organizational goals.
