Psychiatric Care Manager

The Psychiatric Care Manager coordinates and supports the delivery of psychiatric services within Finger Lakes Community Health, managing referrals and facilitating communication between patients, providers, and behavioral health professionals. This role ensures timely follow-up and continuity of care by developing and monitoring individualized treatment plans in collaboration with the integrated care team. The position also assists with psychiatric emergencies and serves as the primary contact for staff and patient concerns related to psychiatric services.
