Psychiatric Consultant supporting a Collaborative Care Model (CoCM) program for skilled nursing and long-term care settings. This non-patient-facing, population-based role provides psychiatric expertise through case review and consultation with Behavioral Health Care Managers and Primary Care Providers. Services are delivered remotely in compliance with CMS CoCM guidelines to guide treatment planning and improve behavioral health outcomes.
