About this role

A ePEP Case Manager-QMHP supports patients with first episode of psychosis through psychoeducational groups, service coordination, and advocacy.

Responsibilities

Facilitate psychoeducational and recovery groups using a specific curriculum
Provide overall service coordination, referral, linkage, and advocacy to patients with varying needs
Participate in CSC team meetings
Follow up assigned patients who missed a scheduled appointment
Provide crisis intervention and make home visits

Masters degree in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention
