About this role

A Housing QMHP provides supportive housing case management services to individuals with mental illnesses who have experienced homelessness or instability in housing. Services include assessment, development of treatment goals, referral, linkage, advocacy, monitoring, crisis intervention, life skills training, and continuity of care.

Responsibilities

Provides overall service coordination, referral, linkage, and advocacy to consumers with varying needs.
Completes ongoing psychosocial assessments for program participants and formsulates individual housing service plans.
Develops measurable objectives and goals agreed upon by the program participant.
Assists in obtaining and modifying goals as needed.
Provides skills training to promote stability in housing (budgeting, self-care, developing natural supports, and independent/instrumental living skills).
