Re-Entry Intake Coordinator

This role conducts re-entry intakes and coordinates release planning for individuals receiving Medication Assisted Treatment (MAT) within the Sheriff's department. The position serves as the primary non-clinical liaison between re-entry services, medical staff, case managers, and community providers to ensure continuity of care. The coordinator develops person-centered re-entry plans addressing housing, treatment, benefits, and recovery supports to facilitate seamless transitions upon release.
