Recovery Case Worker

The Recovery Case Worker provides intensive psychosocial recovery support to individuals with severe and enduring mental health challenges, helping them build stability, independence, and achieve personal recovery goals. This on-site role involves delivering outreach support across the community and working collaboratively with consumers, support networks, and external services to create lasting positive change. The position focuses on developing person-centred recovery plans, managing complex case loads, and ensuring housing stability and reduced hospital admissions.
