Reentry Mentor

The Reentry Mentor serves as a core member of the Transition Planning & Transportation team, providing targeted outreach, engagement, and support to individuals detained in Rikers Island facilities. This role involves facilitating orientations, tracking participant attendance, and guiding eligible individuals into services to promote successful reentry into the community. The position requires maintaining consistent follow-up, supporting communication and scheduling, and accompanying individuals to community providers upon discharge.
