Reentry Services Case Manager

The Reentry Services Case Manager is part of a multi-disciplinary team responsible for conducting in-reach to correctional institutions to publicize, enroll, and case manage clients in the Ralph Gants Reentry Services Program. This role involves building rapport with individuals returning from incarceration to facilitate a warm-handoff and coordinate essential services such as education, career counseling, and housing support. The position requires maintaining monthly contact with clients for 18 months and collaborating with community-based resources to address their needs.
