Reentry Targeted Case Manager

The Reentry Targeted Case Manager provides person-centered, recovery-focused care coordination to justice-involved Apple Health clients during the pre-release period. This role supports individuals transitioning from incarceration to the community by completing reentry assessments, developing care plans, and coordinating medical and behavioral health services. The position ensures compliance with state regulations and Medicaid billing requirements while collaborating with clinical teams and community partners to support continuity of care.
