Referral Coordinator

This role manages, tracks, and monitors outgoing and internal referrals to ensure patient safety, care coordination, and continuity of care. The position assists with healthcare access to manage patient care and acts as a liaison to hospitals, specialty clinics, and other providers outside of the health center. The coordinator ensures timely processing of referrals, maintains accurate documentation in the electronic medical record, and collaborates with clinical teams to coordinate patient appointments.
