Referral Coordinator

The Referral Coordinator processes referrals and coordinates care for designated patients between primary care providers, specialists, and other entities involved in the patient's care. The role involves communicating with providers, patients, and families to ensure the receipt of needed services, reinforce adherence to care plans, and facilitate appointments. The coordinator also works collaboratively with the care team during pre-visit preparation, during patient visits, and between visits to manage referrals and identify gaps in care.
