Registered Nurse - PCTC (CEN)

As a Registered Nurse in the Primary Care and Treatment Centre, you will support the inter-professional team in managing acute and chronic illness for children, adults, and seniors who lack primary care providers. The role involves conducting physical assessments, administering treatments, developing comprehensive care plans, and providing health education to promote wellness and disease prevention. You will also facilitate care navigation and maintain awareness of community-specific social, economic, and cultural conditions to deliver effective patient outcomes.
