Suncrest Hospice
The Registered Nurse (RN) will provide hospice care to patients in their homes, skilled nursing facilities, and assisted living settings as part of an interdisciplinary team. Working Monday through Friday from 12 p.m. to 8 p.m., the RN conducts psychosocial assessments, coordinates with various care professionals, and manages documentation and communication while traveling across the service area.
