The Registered Nurse Case Manager is a part-time nursing role accountable for patients’ discharge planning and for supporting clinical, financial, and resource utilization goals across the acute care continuum. The position serves as a leadership role within the care team and is scheduled 40 hours per bi-weekly pay period with a 6–8 week daytime orientation.
