Registered Nurse (RN) - Hospice

This role involves coordinating and facilitating an interdisciplinary plan of care for hospice patients through comprehensive assessments and skilled nursing services. The position requires traveling to patient homes and facilities to evaluate care needs, collaborate with physicians and multidisciplinary teams, and ensure accurate documentation. The nurse provides direct patient care, including wound care and IV management, while responding to patient and family needs throughout the end-of-life journey.
