About this role
DaVita is seeking an organized, self-directed Registered Nurse to manage Peritoneal Dialysis (PD) patients, blending clinical care, patient education, and case management. The RN will educate patients and caregivers in-clinic, enable independent home dialysis, and coordinate ongoing care and follow-up. The role includes home visits, monitoring treatment progress, and collaborating with interdisciplinary teams to ensure safe, high-quality outcomes.
