About this role
DaVita is seeking an organized, self-directed Registered Nurse to manage Home Hemodialysis (HHD) patients, educating patients and caregivers to perform dialysis independently at home. The role combines clinical care, patient education, case management, and periodic home visits to ensure safe, high-quality outcomes. The RN will collaborate with interdisciplinary teams and provide on-call support as needed.
