Registered Nurse

This role involves collaborating with outreach teams and hospital personnel to identify needs, establish goals, and increase awareness of hospice care services. The position requires determining patient eligibility for hospice or palliative care based on goals of care and communicating the philosophy and benefits of these services to patients and families. The nurse coordinates admissions, ensures adherence to the hospice plan of care, and participates in quality improvement and mandatory training.
