This part-time Registered Nurse role supports an innovative hospital-at-home model by identifying and enrolling appropriate patients into the Home-Based Hospital Care program. The position involves conducting bedside intake, coordinating seamless discharges to home-based services, and serving as a key connector between hospital and home teams.
